the past months we have started to
receive increased requests from
patients who wish to "opt out" from
the NCRS. If this is something you
have considered we would be grateful
if you could read the following

Health.

At present the NCRS has not been
implemented in this area. This
process allows us to know in advance
who has requested to "opt out".
